1. Complete and mail the form with payment to: National Dental EDI Council, 2020 W Indian School Road, Suite F-44. Phoenix, AZ 85015-5040 or submit the form online.
2. If you prefer, we will be happy to take your enrollment by phone (602) 266-7740. To ensure your enrollment is guaranteed, you may choose to pay by credit card when making your reservation or you can mail your payment. Payment must be received by the deadline to receive the discounted rate. Pre-registration deadline is Friday, April 18, 2008. After this date, you must register on-site at the conference.
3. You may also send your registration to us by e-mail at ndedic@ndedic.org. Please include name, title, mailing address, phone and billing information.
4. Registration information must be received by Monday, March 30th for you name and company to be included in our list of registants which will behanded out in the Conference registration packet
If you prefer to fax us your registration, the number is (602) 277-6798. Please include credit card information or mail your payment prior to the advance deadline of Friday, April 15, 2007. After this date you must register on site at the Conference.
Upon receipt of your registration, a confirmation will be forwarded directly to you. In case you have enrolled close to the conference date, and/or your confirmation has not been received prior to your departure, please attend without concern. Your name will be in our records.
CANCELLATION POLICY:
A full refund will be given minus a $75 administrative fee when canceling 30 days or more prior to the start of the Conference. No refund will be given when cancellation is received 29 days or less prior to the start of the Conference. All cancellations must be in writing on company stationery and e-mailed, faxed or mailed to the NDEDIC office or via email with a company return email address.
Special Accommodations: If you or your spouse require special accommodations or you have special dietary needs to maximize your comfort during the conference, please contact NDEDIC to discuss any necessary arrangements.
